We will take down basic information and provide you with the access necessary to begin your paperwork online, from the comfort of your own home. In order to help you make your visit with us even more convenient, we have provided your paperwork below for you to print and complete prior to your first visit. We set aside an hour for your first visit in order to complete a thorough examination and health history with you before providing treatment. We want to make sure we have plenty of time to meet your needs. We look forward to serving you!
